Mod 2012 Spring Registration Dates and Times:
This spring the Federal Way Mod Soccer Club will offer a soccer league for U6 - U10 players. Each team will play 8 games with most games played on Sundays and a few on Saturdays between 12:00 - 4:30 pm beginning April 15 through June 10th with no games played Memorial Day weekend.

Each player will receive a FWMSC T-Shirt. Parents/guardians will need to provide shorts, socks, shin-guards and appropriate shoes.
Registration Procedure
1. Register Online
* Players must register under their legal age bracket, even if 'playing up'.
* If unable to find the automated Player Registration Form, please complete and sign the FWSA Medical Release Form below and give to your coach.
2. Attend one registration date below to complete the registration process. You must attend a registration date for your player to be placed on a team.

3. Bring the following with you to the registration date:

Online registration confirmation for each child – after you have registered online you will receive a registration confirmation in your email. Please print the attachment and bring with you to registration. If you do not receive an email confirmation please email Registrar Novella Fraser at fwregistrar@yahoo.com.
Birth certificate – if your player hasn’t played for Federal Way Mod in the past bring a copy of the player’s birth certificate. We will not keep the birth certificate.
Cash, check or money order - made payable to FWMSC. Scholarship request are also available. Scholarship form is located below
Scholarships
While the Clubs rely on player fees and fundraisers to pay for their expenses, in the event a player cannot afford the Club registration fees, assistance may be available to those who qualify.
Any family requesting a scholarship for their child's player registration fees, must fill out this Mod Scholarship Request Form and turn it into the Club at the time of registration.
Refunds
Prior to first game will receive registration refund minus $15.00. After first game - no registration refund.
Referee Clinic Reimbursement
The Federal Way Mod Soccer Club (FWMSC) will cover instruction and test fee for all players, parents, and coaches of FWMSC teams. For more information please click on Mod Referee Clinic Reimbursement Instructions for more information.

Staff Registration
Thanks for your interest in volunteering for Federal Way Mod Soccer! Our organization runs on the dedication of hard-working volunteers like you. Anyone working with our youth players or handling team funds must register online through the Bonzi system and be Risk Management Approved (RMA). This includes, but is not limited to, a Coach, Assistant Coach, Trainer, Manager, and Treasurer. An electronic confirmation of your registration will be sent to you.
Returning Users:
Click on the Staff Registration link and then login into your existing Member Account. If you've forgotten your information, contact the appropriate Registrar.
New Users:
Click on the Staff Registration link and then create a Member Account, completing all requested data.

Code of Ethics
The Coaches' Code of Ethics form should be signed by every head coach indicating that they have read and will abide by the WSYSA Code of Ethics. This signed form is to be turned in with their team registration packets. The Player's Code of Ethics and the Parent's Pledge to be a Good Sport are tools for the coach, if desired.
