Registration Fees
* Every player (including coach/multiple child) is required to participate in the Federal Way Mod Soccer Club Fundraiser. This consists of selling a minimum of TBA. In lieu of the fundraiser, you may choose to donate $30.00. The registration fee is given to your team, who will turn it into the Club.

Returning Users:
Click on the above green Player Registration link and then login into your existing Member Account. If you've forgotten your information, contact the appropriate Registrar.
New Users:
Click on the above green Player Registration link and then create a Member Account, completing all requested data. Once completed, a confirmation email will be sent to you, with the player registration form as an attachment. Print the form, sign it, and give it to your coach.
If your child is new to FWSA, a copy of his/her proof of age document, i.e. birth certificate, passport, etc., will be required. They cannot play until this is accomplished. If the player isn't currently associated with a team a Coordinator will contact you once you've registered on-line. Be sure to retain the player registration form for future team placement.
Notes:
- Players must register under their legal age bracket, even if 'playing up'.
- If unable to find the automated Player Registration Form, please complete and sign the FWSA Medical Release Form below and give to your coach.

To save time at tryouts, please bring this completed form. Even if you choose not to participate in tryouts, you're welcome to complete and drop one off for priority placement on a fall team.

Scholarships
While the Clubs rely on player fees and fundraisers to pay for their expenses, in the event a player cannot afford the Club registration fees, there are scholarships available. Please contact your team coach, manager, or treasurer for information on how to apply.
United Scholarships - Contact the FWU Treasurer, Darlin' Boudreau for information or an application. Information submitted with the application is kept strictly confidential.
Mod Scholarships - Any family requesting a scholarship for their child's player registration fees only, must give their coach a letter explaining their hardship in place of their registration fees. This player must also participate in the Club fundraiser.
2007 Summer Camps Information & Form
Our camp program is operated for the players of the Federal Way United Soccer Club (FWUSC) and guests. FWUSC Director of Coaching Rob Walker has put the camp schedule and curricula together so that camp attendees will be challenged and enjoy the experience. FWUSC sees the Summer Camp Program as a time to practice hard, play hard, and have fun as part of the overall camp experience. Coach Walker and staff bring many years of coaching experience forward and believe that a good camp program is an important anchor to an all-around soccer development program. United Referee Clinic Reimbursement
The Federal Way United Soccer Club (FWUSC) will cover the initial cost of a Grade 8 or Grade 9 Referee Clinic fee up to $50 for all players and coaches of FWUSC Teams.

Staff Registration

Anyone working with our youth players or handling team funds
must register online through the Bonzi system and be Risk Management Approved (RMA). This includes, but is not limited to, a Coach, Assistant Coach, Trainer, Manager, and Treasurer. An electronic confirmation of your registration will be sent to you.
Returning Users:

United Coaching Application
In addition to registering online, all United coaches and prospective trainers are required to submit a FWUSC Coaching Application. No coaching application is required for Mod.

This code of ethics has been developed to clarify and distinguish approved and accepted professional, ethical and moral behavior from that which is detrimental to the development of soccer within the Washington State Youth Soccer Association (WSYSA).

United Team Registration
All FWUSC teams are required to submit a Team Registration Form, along with one team check for all player registration fees. All FWUSC teams must maintain a team bank account with the bank designated by the Club. Contact the FWUSC Treasurer, Darlin' Boudreau for further information.

Tournament Roster Form
Due to sensitive information, rosters from the Team Registration form must not be used for tournament entry purposes. A specific tournament roster form has been created for your convenience.

Please submit this form when requesting payment from the FWUSC Treasurer.

Please submit this form when giving the FWUSC Treasurer money to deposit.
Paid Coaches, Trainers, etc.
IRS regulations require we have a form W-9 on file for any paid person or entity (business). Before a team pays anyone, other than for reimbursement of prepaid team expenses, a W-9 form must be completed and given to the FWUSC Treasurer. This form is available at http://www.irs.gov/pub/irs-pdf/fw9.pdf.
Further, the hiring of any coaches and/or trainers must be coordinated through our Director of Coaching, Rob Walker.
Opening a Team Bank Account
All FWUSC teams must open a team bank account with Bank of America on 320th and Pacific Highway. Please contact the Club Treasurer for further information.
Mod Team Registration
Code of Ethics
The Coaches' Code of Ethics form should be signed by every head coach indicating that they have read and will abide by the WSYSA Code of Ethics. This signed form is to be turned in with their team registration packets. The Player's Code of Ethics and the Parent's Pledge to be a Good Sport are tools for the coach, if desired.
